Seeking a full-time Psychiatric Mental Health nurse practitioner with experience to join Providence ElderPlace's PACE program. In this role, you'll assist an established team providing comprehensive care, social services, psychiatric care and acute are to geriatric patients across the Portland Metro area. Patient visits occur in facilities, clinics and patient homes. This position will also assist in integrating behavioral health into PACE practices and will work closely with LCSW Mental Health Case Managers.
